Individualized Home Supports (IHS) is a waiver service designed to help people live successfully and independently in their homes and communities. Services are available to eligible individuals enrolled in Minnesota Home and Community-Based Services (HCBS) waiver programs. IHS services are tailored to each person’s assessed needs, goals, and preferences.

SmartChoice Home HealthCare provides the following Individualized Home Supports services:

• Individualized Home Supports without Training
• Individualized Home Supports with Training
• Individualized Home Supports with Family Training

These services may include support in areas such as:

• Community participation and engagement
• Health, safety, and wellness
• Household management
• Medication reminders and routine support
• Assistance with activities of daily living (ADLs)
• Behavioral and positive support strategies
• Increasing independence and daily living skills
• Building and maintaining social connections

Individualized Home Supports Without Training

This service provides ongoing support and supervision to help the person maintain independence and safely live in the community. Staff assist with daily routines, community access, household tasks, health and safety needs, and other support identified in the person’s service plan. The focus is on providing support rather than teaching new skills.

Individualized Home Supports With Training

This service provides training and instruction to help the person develop, improve, or maintain skills needed for greater independence. Staff work directly with the person on goals such as cooking, budgeting, personal care routines, transportation use, communication, social skills, and community participation.

Individualized Home Supports With Family Training

This service provides training and support to the person’s family members or caregivers so they can better assist the individual in achieving their goals and maintaining stability at home and in the community. Training is individualized to the person’s needs and may include guidance on routines, behavioral supports, communication strategies, safety, and skill development.
