Chapter 245D
Chapter 245D Services encompasses a wide array of services that revolve around two different kinds of care: Basic and Intensive care. SmartChoice Home Healthcare provides both basic and intensive services.
Our basic services include:
- Homemaker services
- Individualized home support
- Individual community living supports (ICLS)
- Night Supervision
- Respite Care
Our intensive services include:
*Eligibility for Chapter 245D programs require a waiver that certifies a condition before we are able to provide 245D services. Acquiring a waiver will require approval from the State of Minnesota before we are able to provide the service.
